A five-doctor practice refers out several times a day and finds out what happened by asking the patient at the next visit, if there is one.
A clinic can raise a referral, see when the patient was seen, and get the report back against the original reason. No fee is paid for sending a patient.
Where it goes wrong here

The referral never became an appointment
The slip was issued, the booking was left to the patient, and nobody checked whether it happened.
The specialist's report went to the patient, not the clinic
So the treating doctor is the last to know.
The patient never returned
Referring out is how a clinic loses a patient it had, when the loop is open.

What nobody gets paid
No fee, commission, rebate or revenue share reaches anyone for sending a patient — in either direction. Fee-splitting for referrals is prohibited under IMC (Professional Conduct) Regulations 2002 cl. 6.4 and the NMC Professional Conduct Regulations 2023, and the exposure sits on the practitioner's registration.
