Four levels of fix

Why level two feels like enough
It works immediately and visibly. The person who made the error stops making it, the deduction disappears from the next few claims, and the matter closes. Then the shift changes, or the person moves, and the cause is intact because nothing outside their memory ever changed.
The test
Ask whether a new employee, on their first day, with no briefing, would reproduce the deduction. If the answer is yes, the fix was applied at level one or two. Levels three and four are the ones that survive turnover, which is the only durability that matters in a hospital.
