The lead-time relationship

No-show risk rises with booking lead time, which is why an appointment made six weeks out behaves differently from one made yesterday. That is not a reason to refuse advance booking; it is a reason to reconfirm long-lead appointments specifically rather than treating every slot the same.

The measure that changes behaviour

Count no-shows by lead time and by slot, not as one clinic-wide percentage. A single number tells you there is a problem. The breakdown tells you it is concentrated in Monday morning slots booked more than three weeks ahead, which is something you can actually act on.
